- Clinical history:
A 70-year-old man was admitted for muscle weakness and rectal prolapse. The patient was found to be hypokalemic. A sigmoid colon tumor was found and subsequently removed. - Pathology:
A 15 cm segment of bowel was excised to reveal a large sessile tumor with a smooth surface. The tumor measured 11 x 7 cm. in diameter and extended to within 2 cm. of the distal resection margin. The mucosa was otherwise normal. The serous surface was not prominent. Histological examination confirmed the presence of a villous adenoma. - Additional information:
Occasionally, villous adenomas secrete large amounts of mucoid protein and/or potassium-rich fluid, sufficient to cause hypoalbuminemia or hypokalemia. Villous adenoma is the least common but most dangerous type of adenomatous polyp. Invasive carcinoma is detected at resection in up to 301% of cases.
