An 82-year-old woman presented with an episode of melaena (dark, tarry stools). She had a 6-month history of dyspepsia and nausea. She had recently noticed weight loss and early satiety. Shortly after admission, she experienced a major episode of melaena and died. - Pathology:
This is a postmortem specimen, sectioned to provide a sagittal view of the gastrointestinal tract, stomach, proximal duodenum, and pancreas. A large 7×5 cm ulcer is clearly visible in the lesser lobe of the lesser arc of the stomach. The ulcer is shallow and wide, with raised, rounded margins and a necrotic boxed base. Loss of gastric sigmoidity extends from the ulcer. Dissection of the ulcer reveals a raised margin of pale, homogeneous tumor material. Two eroded arteries with recent hemorrhage were found at the base of the ulcer. Part of the pancreas is adherent to the serosal surface of the ulcer. Histological specimens taken from the ulcer (areas seen as regular 3 cm defects) showed that it is an opalescent, well-differentiated gastric adenocarcinoma with direct invasion of the pancreas.
