  • Clinical history:
    A 54-year-old man presents with rib pain. He is an active intravenous drug user. Further questioning reveals a history of intermittent hematuria, fever, malaise, and vomiting. Examination reveals hypertension and fever. The surrounding area shows Janeway spots on the extremities and traces of recent IV drug use. A systolic murmur is heard on chest auscultation. Blood tests show elevated inflammatory markers, impaired renal function, elevated LDH, and multiple bacterial blood cultures. Echocardiogram shows a large motile tricuspid vegetation. He was initiated on treatment for infective endocarditis but later died of sudden cardiac arrest.
  • Pathology:
    This is a postmortem examination of a patient's kidney. The kidney is divided into two halves, and the half-cut surfaces are shown. Several well-defined areas of clinically white are visible in the cortex. The base of these pyramids is at the cortical surface and along the renal pillars, and the apex points toward the middle. The largest is seen in the upper lateral part of the kidney. These light areas are infected patches of renal tissue. There are dark, irregularly shaped areas that are formed by areas of hemorrhage.
