  • Clinical History:
    A 40-year-old man presented with dyspnea and lower extremity swelling. He is a smoker and has diabetes and a history of myocardial infarction. Examination revealed bilateral pleural effusion, peripheral pitting edema, and swollen testicles. Light transillumination of the swelling showed red light. Radiographic evidence of heart failure was evident on radiographs. He was treated for heart failure but later died on admission.
  • Pathology:
    The specimen is a testis and its coverings cut transversely to expose the cut surface. The cavity, bounded by the inner and outer layers of the tunica vaginalis, has become dilated due to the accumulation of fluid. This is an example of a hydrocele resulting from generalized edema due to heart failure.
  • Additional Information:
    A hydrocele is an accumulation of serous fluid between the parietal and visceral layers of the tunica vaginalis surrounding the testicles. A hydrocele can be described as communicating with the abdominal cavity or non-communicating with the abdominal cavity.
