  • Clinical history:
    A 68-year-old man presented with fever and chills. A detailed examination revealed a 6-month history of recurrent bilateral rib pain and hematuria. Biochemical tests showed significantly impaired renal function with normal serum calcium. Abdominal CT scan showed bilateral hydronephrosis with multiple renal stones, as well as perirenal and subphrenic purulent masses. The patient subsequently died of progressive renal failure.
  • Pathology:
    This is a patient's kidney that is partially divided. The pelvi-calyceal system is markedly dilated. Significant atrophy of the renal tissue can be seen, in some places reduced to the smallest frame. A large spotted brown-white stone lies in the pelvis, and a smaller stone has obstructed the lumen of the ureter. The ureter upstream of the obstructed stone is dilated. Numerous stones in the pelvis are visible in the specimen.
