Clinical history:
A 60-year-old man had a history of four episodes of severe, convulsive abdominal pain over the past year, each lasting two hours and associated with meals. He presented after a similar attack associated with vomiting and fever. This last attack did not resolve spontaneously, and he underwent cholecystectomy.
Pathology:
The thick gallbladder has been opened to show a thickened hemorrhagic mucosa and numerous irregular, faceted figures. A large stone is lodged in the neck of the gallbladder. The serous surface of the gallbladder is convoluted and has lost its usual luster. This is an example of cholelithiasis complicated by cholecystitis.
