• Clinical history:
  A 72-year-old woman presented with colicky rib pain and ongoing weakness. Recurrent hematuria was noted. Biochemical tests showed significant renal dysfunction. Abdominal CT scan showed left renal insufficiency and right renal hydronephrosis and hydroureter due to a smaller stone obstruction. Percutaneous lithotomy was attempted to remove the obstruction, but the patient died of a cardiac event during the procedure.
• Pathology:
  This is the patient's right kidney, which is partially divided. The pelvi-calyceal system is markedly dilated, and the renal tissue, especially in the cortex, is significantly atrophied. In the renal pelvis, near the ureter-pelvic constriction, there is a large brown stone.
