- Clinical history
- A 50-year-old alcoholic was admitted with 2 weeks of weakness and shortness of breath.
- During his illness, he reported a productive cough, chest pain, and hemoptysis.
- Examination revealed fever, cyanosis, lethargy, and respiratory distress.
- The morning after his death, a spinal tap was performed, which yielded a green opalescent fluid.
- Streptococcus pneumoniae grew in blood culture.
- Pathology
- This specimen is an example of ventriculitis, and pneumococcal meningitis and right basal lung pneumonia were also found after autopsy.
- A horizontal section through both sides of the brain shows both lateral ventricles.
- The ventricles show a thick, rough line of ependymal covering with accumulation of cellular detritus around the choroid plexus and also in the anterior horn.
- Pathology showed extensive neutrophil infiltration under the meninges, as well as various local severe (sub)endothelial infiltrations with vascular mucosal occlusion and vascular wall damage.

- Ventriculitis is a rare complication of intracranial infection.
- In adults, it more often occurs as a secondary complication after surgical intervention/instrumentation or trauma.
- Neonates under 6 months of age have a higher incidence of ventricular infection.
- Diagnosis depends on laboratory examination of CSF and imaging, especially using CT and MRI.
