  • Clinical History:
    A 16-year-old boy presented with a 3-month history of increasingly swollen and painful right knee. On examination, a painful swelling was felt above the right knee. Blood tests revealed an elevated alkaline phosphatase level. A knee radiograph showed periosteal reactive changes in the distal femur, suggesting a bone malignancy. The patient then underwent a resection of the right femur before chemotherapy. He made a full recovery.
  • Pathology:
    This specimen is a distal femur resected from a patient. The cut surface shows a large pale infiltrated tumor, 10 cm in size, extending through the periosteum proximal to the joint surface. This is an osteosarcoma of the femur.
  • Additional Information:
    Osteosarcomas are malignant bone tumors characterized by the production of osteoid matrix or unabsorbed bone. They are the most common primary bone malignancy. Most cases occur in the distal femur, and the most commonly affected regions are the groin and shoulder joints. Men are more commonly affected than women. They occur in a biphasic age distribution, with the majority occurring in childhood and adolescence before the age of 20 and a second peak in older age, over 60 years.
