  • Clinical history:
    A 61-year-old man presented with anginal chest pain and shortness of breath. He had had these symptoms for 6 years, worsening each year. Examination revealed cyanosis and tachycardia with a weak pulse. Swelling was noted on the right side of the neck. A tremor was felt over his heart. A loud systolic and diastolic murmur was heard over the aorta. Chest radiographs showed cardiomegaly with a large round lesion in the right upper mediastinum, continuous with the cardiac shadow, with radiographic evidence of cardiogenic failure. Blood tests were positive for antitreponemal antibodies. The patient's condition deteriorated and he died of heart failure.
  • Pathology:
    This is a patient's enlarged heart, including the aortic arch and descending aorta. The ascending aorta is dilated to 7 cm in diameter, and there is a large aneurysmal dilation measuring 11 x 13 cm in diameter. It is opened to reveal the wrinkled, damaged intimal surface. Intimal atheroma is also noted. The descending, left common carotid, and post-lateral arteries are diverted to the left through the aneurysm. There is a banded thickening 5 mm high in the interior of the aneurysm. This is the site of the externally attached pericardial sac. There is marked occlusion of small vessels in the aortic adventitia. This is a syphilitic aortic arch aneurysm.

    Syphilis is a chronic infection caused by the spirochete Treponema pallidum. The infection is most commonly transmitted sexually, but it can also be acquired congenitally, through placental transmission. People at risk for syphilis infection include sexually active individuals, drug users, HIV-infected patients, and men who have sex with men. Syphilis infection rates decreased significantly after the introduction of penicillin in 1943; it remains the mainstay of treatment today. However, since the 2000s, infection rates have begun to rise again.
