  • Clinical history:
    A 55-year-old woman presents with severe dyspnea, productive cough, and oral candidiasis. She is immunosuppressed and has a history of rheumatoid arthritis, which was treated with steroids and cyclophosphamide. Stool cultures grow staphylococci. She was started on appropriate therapy upon admission, but died shortly thereafter.
  • Pathology:
    The right lung has been dissected. Several irregular abscess cavities are visible. The largest of these, located at the top of the lower lobe, is 4 x 3 cm in diameter. At the top of the upper lobe is another irregular abscess cavity, less conspicuous, about 3 x 2 cm in diameter, surrounded by a zone of consolidation. Several small abscesses are also visible. The middle lobe has patchy consolidation. Many of the bronchi are congested and blocked with purulent masses. Cultures taken from the specimen grow Staph. aureus. This is an example of how an immunosuppressed patient can develop multiple Staphylococcus abscesses in the lungs.

    Staphylococcus aureus is a gram-positive cocci. It is part of the human microbiota, usually found on the skin or upper respiratory tract. It is usually commensal but can cause potentially contagious infections such as skin infections, often or, rarely, pneumonia and endocarditis. It can cause both community-acquired and hospital-acquired pneumonia. Hospital-acquired staph pneumonia is most commonly associated with intubation and prolonged hospitalization. The incidence of hospital-acquired pneumonia caused by methicillin-resistant Staph Aureus (MRSA) is increasing.
