A 38-year-old woman presents with severe nausea, vomiting, fever, and chills. She has a history of recurrent urinary tract infections over the past 6 months. She has required several courses of oral antibiotics and one visit with IV antibiotics. Blood tests show elevated inflammatory markers. Urinalysis is positive for white blood cells (WBC). CT scan shows unilateral hydronephrosis and pyelonephritis of the left kidney. She fails to respond to conservative treatment and undergoes nephrectomy. She makes a full recovery. - Pathology:
This is a specimen of a nephrectomy of the patient's left kidney. The kidney is cut to expose the cut surface. The pelvis and calyces are very dilated and filled with yellow pus. There is significant fibrosis in the renal parenchyma. In the medial zone, approaching the lateral margin, there is a hemorrhagic necrotic area 35 x 12 mm in diameter, filled with pus. Two similar small hemorrhagic necrotic areas are visible on the capsular surface. These lesions are probably continuous with the lesion visible on the cut surface, and very likely caused hemorrhage into the abscess cavity. This lesion would have caused a perinephric abscess.
