- Clinical History:
A 60-year-old woman presented with 12 months of pain and increasing swelling in her right shoulder. On examination, a nodule was palpable over the right shoulder. There was limited abduction and external rotation of the shoulder joint. No palpable lymph nodes were present. A shoulder x-ray showed a mass impinging on the superior scapula above the glenohumeral joint. The nodule was biopsied and the scapula was completely removed. - Pathology:
This specimen is a resected right scapula from a patient. An irregular lobular tumor with a maximum diameter of 11 cm arises from the scapular fossa and has extended to involve the acromion and the scapular process. The tumor is multi-colored with a pale yellow to brown hue with patchy surface hemorrhage. There is some adherent muscle and fibrous tissue. The mass has infiltrated and replaced normal bone. Histologically, the tumor is composed of pleomorphic round and spindle-shaped cells with a high number of mitotic figures and cartilage formation. It is a chondrosarcoma of the shoulder. - Additional Information:
Chondrosarcomas are malignant bone tumors that produce cartilage. They are the third most common type of primary bone malignancy after myeloma and osteosarcoma. Conventional tumors are the most common subtype of chondrosarcoma, accounting for 90% cases. Less commonly diagnosed subtypes include clear cell, differentiated, and mesenchymal chondrosarcomas.
