  • Clinical history:
    A 36-year-old woman was admitted to the hospital with a 3-week history of bloody diarrhea and lower abdominal pain. Additional history taking revealed 4 similar episodes of diarrhea and abdominal pain over the past 7 years. Sigmoidoscopy showed an erythematous, ulcerated, and swollen rectal mucosa. She was started on steroids, but her symptoms did not improve. She passes the entire colon.
  • Pathology:
    The resected segment of colon is cut open to expose the mucosal surface. There are large, confluent focal areas of ulceration separated by swollen islands of residual mucosa. The ulcers have necrotic bases with raised margins, some of which form “pseudo” polyps. Histology of the proximal intestinal mucosa showed acute inflammatory changes with crypt abscesses, localized necrosis, and ulceration. This is an example of acute ulcerative colitis (UC).
  • Additional information:
    Ulcerative colitis is a chronic ulcerative-inflammatory disease that usually affects the rectum and can spread to other parts of the large intestine. The inflammatory process is diffuse, but usually limited to the mucosa and superficial submucosa. The causative agent of inflammatory colitis is unknown. It usually begins between the ages of 15 and 25 and is slightly more common in women.
