- Clinical history:
A 60-year-old man was admitted with jaundice, melena, and abdominal distension. He had a history of untreated hepatitis C infection acquired through previous intravenous drug use. Further investigations revealed a 9-month history of significant fatigue, weight loss, nausea, and intermittent right upper quadrant heaviness. Liver ultrasound revealed two large hepatic masses. The patient died shortly after admission from suspected esophageal variceal bleeding. - Pathology:
This is a liver specimen from a patient after death. The cut surface of the liver has a multinodular appearance consistent with macronodular cirrhosis. These multiple nodules vary in size up to 2 cm in diameter and are separated by narrow fibrous tissue. Two large round tumors are also visible. They are 8 cm and 6 cm in diameter with a heterogeneous cut surface due to local necrosis, hemorrhage, and bile staining. This is an example of hepatocellular carcinoma arising in a cirrhotic liver.
