A 64-year-old man presented with a 5-month history of general malaise, weight loss, and dull right costal pain. Examination revealed a palpable right abdominal mass. He was noted to have hypertension. Urinalysis revealed microscopic hematuria. The patient underwent a right nephrectomy. - Pathology:
This is a kidney that has been partially excised in the plane of the cone and mounted to show the cut surface. The lower pole of the kidney is replaced by a round, ill-defined, irregular mass, 5 cm in diameter, which compresses and distorts the upper renal parenchyma. The cut surface of the tumor has a variable appearance caused by areas of hematoma and necrosis. Several small pale yellow tumor nodules are seen in the cortex and medulla above and separate from the tumor at the lower pole. These are intrarenal metastases. The renal pelvis appears mildly dilated, with a few renal pilonidal cysts, indicating a degree of hydronephrosis. The capsular surface is finely nodular with a few coarse scars and contains a few small simple cysts (see posterior side of specimen). Histologically, the tumor was diagnosed as renal cell carcinoma.
