- Clinical History:
A 66-year-old man presents with epigastric pain. He is deaf and unable to speak. Examination reveals a tender epigastrium and several nodular tender nodules on the forehead and scalp. Blood tests show low hemoglobin, impaired liver function, and a positive anti-treponemal antibody test. After admission, he experienced major gastrointestinal bleeding and death despite intervention. - Pathology:
This specimen is a section of the patient's head. On the outer surface to the left of the midline, there are several well-defined necrotic nodules in the parasagittal region. The nodules are brown in color and up to 3-4 cm in maximum diameter. The nodules erode the outer part of the skull, and the opposite periosteum is thickened with fibrinous inflammation. These nodules are chronic syphilitic nodules or scaly cheek nodules, which are characteristic of favorable stages of tertiary syphilis. - Additional Information:
Syphilis is a chronic infection caused by the spirochete bacterium Treponema pallidum. It is most commonly a sexually transmitted infection, but it can be acquired during childbirth by transplacental transmission of the bacteria. Those at higher risk of infection include those of sexually active age, intravenous drug users, HIV-infected patients, and homosexual men. Syphilis infection rates decreased significantly with the introduction of penicillin in 1943, which remains the mainstay of treatment. However, since 2000, the initial infection rate has been increasing.
