  • Clinical History:
    An 80-year-old man presented with hematemesis. He was known to have alcoholic cirrhosis with esophageal varices. Examination revealed multiple spider veins and a large amount of abdominal ascites, as well as testicular swelling. The swelling was bright red with light. After admission, he experienced another large amount of hematemesis and died shortly thereafter.
  • Pathology:
    The specimen consists of the testis, tunica vaginalis, and distal end of the spermatic cord. The testis and its surrounding layers are divided to allow for visualization of the incision. The tunica vaginalis is thickened and the recruited cavity is dilated. The testis is normal. This is an example of a chronic secondary communicating hydrocele.
  • Additional Information:
    A hydrocele is an accumulation of serous fluid between the parietal and visceral layers of the tunica vaginalis surrounding the testicles. A hydrocele can be described as communicating with the abdominal cavity or non-communicating with the abdominal cavity.
