- Clinical history:
A 40-year-old man presented to his family physician with a 2-week history of hematuria and new-onset headaches, accompanied by blurred vision. The family physician noted a blood pressure of 260/110 and referred the patient to the hospital. The patient collapsed on arrival. A CT scan of the head revealed a large subarachnoid hemorrhage from a ruptured berry aneurysm. The patient died shortly after admission. - Pathology:
The specimen is a large kidney. The renal parenchyma is almost completely replaced by numerous dilated cysts of varying size, up to 3 cm in diameter. The cysts have thin translucent walls, and some cysts contain a material of different colors, giving a “marbled” appearance to the cut surface of the kidney. The different colors are due to secretions within the cysts, which may be mixed with hemorrhage. The external surface appears lobular due to the numerous protruding cysts. Any remaining renal parenchyma is severely atrophied due to the pressure of the multiple cysts. This is an example of adult polycystic kidney disease.
