An 85-year-old man presented with urinary retention due to benign prostatic hyperplasia. On evaluation, he was jaundiced with cholestatic liver function tests. He underwent transurethral resection of the prostate but died of pneumonia 5 days postoperatively. - Pathology:
This specimen is a lobe of the liver, fixed so that the cut surface can be seen. The capsule is slightly thickened, and the liver material is finely nodular. The intrahepatic bile ducts are dilated. When viewed from the posterior or inferior surface, an irregular pigmented stone, 10 mm in diameter, is seen lodged in a dilated hepatic bile duct. Another smaller stone, 3 mm in diameter, is displaced. This specimen is an example of secondary biliary cirrhosis resulting from obstruction of the large ducts by a liver stone.
