  • Clinical history:
    A 66-year-old woman presented with a sudden onset of severe central abdominal pain, relieved by bending the knees. She passed stools containing mucus and blood ("like redcurrant jelly"). On examination, a mass was noted in the left hypochondrium that hardened with each spasm of pain. The specimen was excised at laparotomy.
  • Pathology:
    The specimen is a small intestine, approximately 20 cm long, with a 2 cm wide posterior abdominal intestinal plug attached (the specimen with the uncut portion is more clearly visible). Approximately 5 cm from the proximal surgical resection margin (which is in the specimen's left hand), a 3 cm diameter polypoid tumor has invaginated into the intestinal mucosa and has been pushed distally, forming an intussusception, 13 cm long. The tumor is seen at the top of the intussusception (near the specimen's right hand). The conjunctiva and exudate seen on the mucosal surface of the intussusception are features associated with early ischemic necrosis. In this case, the histological diagnosis was not recorded; however, the macroscopic appearance is consistent with metastatic malignancy, although the possibility of a primary tumor cannot be unequivocally excluded.
