A woman who had swallowed a bone at lunchtime developed massive hemorrhage later in the afternoon. At laparotomy, the stomach was full of fresh blood, but the cause was not determined. She died one day later, and necropsy revealed a connection between the aorta and esophagus. The stomach was distended with blood and contained several bone fragments. - Pathology:
The specimen is a cutaway of the distal tracheal block (posterolateral to the right edge) and the aortic arch (opened in the anteroposterior plane and in the ocular perspective) and the esophagus (posteriorly and opened in a longitudinal section). The esophageal mucosa is ulcerated and bleeding. A small blue speculum bulb identifies a fistula between the esophagus and the posterior wall of the thoracic descending aorta. - Note:
Although this situation was the cause of traumatic esophageal-aortic fistula, it should be noted that there are also non-traumatic causes of this type. In fact, these fistulas can be caused by pressure from aortic dilation, advanced gastrointestinal malignancies, or aortic implantation into adjacent gastrointestinal segments, and can occur in any part of the aorta.
