  • Clinical history
    • On the Friday before admission, this 38-year-old woman suddenly developed pain behind her right eye associated with the development of chronic weakness in her left leg. Examination revealed the presence of left hemiparesis in a confused hypertensive woman. A case of right homonymous hemianopsia and weakness of the tenth cranial nerve was also noted. There was a clonus in the left leg, and the left foot reflex was elevated. Spinal puncture showed increased pressure and the fluid was blood-stained. Angiography showed an intracerebral aneurysm. She was scheduled for surgery, but the patient died suddenly the day after surgery.
  • Pathology
    • The specimen shows the surface of the base of the brain. At the junction of the right internal carotid and posterior common arteries there is a spherical aneurysm, 5 mm in diameter, which has ruptured. There is subarachnoid blood adjacent to the cerebral carotid artery and the surface of the right anterior ventricle. There is a similar unruptured aneurysm on the left side. The right anterior ventricle appears softer and more fragile anteriorly.
  • Additional information
    • Posterior common artery aneurysms are the third most common aneurysms in the circle of Willis and can cause compression and pathological palsies of adjacent cranial nerves, such as the sixth in this case. The proximity of the ophthalmic portion of the trigeminal nerve to the ruptured aneurysm in this patient may explain the sudden onset of pain “behind the eyes.” The visual field defect is most likely related to compression of the right optic tract. The clinical signs of stroke are a consequence of the effects on the transverse areas of the brain where the blood supply is compromised by the ruptured aneurysm.
