  • Clinical history
    • A 29-year-old man presented with headaches and blurred vision of 22 months duration.
    • Examination showed bilateral hemianopsia and left VI nerve palsy.
    • Skull X-rays showed that most of the sphenoid body is eroded, while some of the dorsum sellae and anterior clinoid process are preserved.
    • Carotid angiography showed overweight and lateral repositioning of the blanket and middle cerebral arteries.
    • Pneumoencephalography showed elevation of the lateral and third ventricles from below.
    • A craniotomy was performed, but the patient died immediately afterwards.
  • Pathology
    • The brain sample is cut horizontally at the right falx cerebri, which remains in situ.
    • The completely replaced pituitary gland is a round tumor with a maximum diameter of 4 cm.
    • The cut surface of the tumor is pale brown and homogeneous (except for an area of hemorrhage at the top, likely caused by surgical trauma).
    • The tumor caused the midbrain to lift.
    • Tumor erosion has destroyed the sphenoid bone; therefore, the sella turcica is enlarged.
    • The optic chiasm is compressed by a tumor.
    • Histologically, this tumor was a chromophobe adenoma originating from the anterior pituitary.

    • This sample is from an old case, and the testing procedures used would be considered ancient.
    • Modern investigations now include an initial CT scan of the brain followed by an MRI scan of the head to better visualize the pituitary focus before any surgical intervention.
