He was 2 weeks postoperative for duodenal ulcer repair. He was hypotensive, hyperkalemic, and hyponatremic with a purpuric rash. His blood cultures were positive for Escherichia coli. He did not respond to treatment and died of septic shock shortly after admission.<\/li>\n<li>Pathology:<br \/>\nThe fused kidney and adrenal gland were fixed to show the cut surfaces. There was extensive hemorrhage within the adrenal medulla, with some blood spilling into the periadrenal fat.
