She reported a history of fever and joint pain in her childhood following a sore throat. On examination, she was cyanotic, with a pulse suggestive of supraventricular fibrillation, elevated jugular venous pulse, a pan-systolic murmur at the base, hepatomegaly, and dependent edema. She was treated with digoxin, Lasix (furosemide), and penicillin, but died of cardiac arrest.<\/li>\n<li>Pathology:<br \/>\nThe specimen is a heart opened to reveal the left atrium and left ventricle. The mitral valve is cut away, but these parts are visible and show significant thickening. The left atrial wall shows blood and fibrin deposition. The left atrial appendage is filled with a blood clot caused by supraventricular fibrillation. The mural thrombus on the atrial wall is in the usual location: irregular prominences called McChallum&#039;s planes have formed in the deep layers of the endocardium (arrows).<\/li>\n<li>Additional information:<br \/>\nThis patient&#039;s history of fever and joint pain following a sore throat strongly suggests a history of rheumatic fever. Rheumatic fever is an inflammatory disease that can affect the heart, joints, skin, and brain.
